Description
Patient Aligned Care Teams (PACTs) within the Veteran's Affairs system are required to manage national performance measures, specifically related to patients with Ambulatory Care Sensitive Conditions (ACSCs). This project focuses on the prevention and progression of renal complications for patients with diabetes by performing urine microalbumin assessments in alignment with the National Kidney Foundation's (NKF) evidence-based practice algorithm. Anticipated findings are increased microalbumin patient orders, timely completion of specimen collection, and follow up collaboration with PACTs lead medical provider, resulting in improved quality of life for patients with diabetes.
